Records Request

You may access your records in your patient portal by selecting the "patient portal" link below and following the prompts. Your portal will no longer be accessible after April 30, 2025. Please retrieve any records before this date. 

You can also select the "Request Records" link then print, sign, and mail the form to our address; Be sure to address it to: Okeechobee Family Practice.

Lastly, you can sign a release at your new physician's office to request records to be faxed on your behalf.
